By participating in this Sweepstakes, each participant agrees to abide by and be bound by these Terms & Conditions and all decisions made by Michael Alexander Optical Boutique, which shall be final and binding in all matters relating to this Sweepstakes.

2. ELIGIBILITY
The Sweepstakes is open to legal residents of New York State who are eighteen (18) years of age or older at the time of entry. Employees, affiliates, immediate family members, and household members of Michael Alexander Optical Boutique may be excluded from participation at the discretion of the Sponsor.
3. OBJECTIVE OF SWEEPSTAKES
The purpose of this Sweepstakes is to promote community engagement, increase awareness of Michael Alexander Optical Boutique within the local community, and introduce participants to the products and services offered by the boutique.
4. HOW TO ENTER
Participants must complete the official Sweepstakes entry process, which may include but is not limited to:

Liking, sharing, reposting, or engaging with promotional content
Completing the official questionnaire or registration form on the company website
Providing accurate contact information
Limit one (1) entry per person unless otherwise stated.
Any incomplete, false, duplicate, fraudulent, or misleading entries may be disqualified at the sole discretion of the Sponsor.
5. PRIZE DETAILS
One (1) winner will receive products and/or services provided by Michael Alexander Optical Boutique with an approximate retail value exceeding Seven Hundred Dollars ($700.00).
Prize may include eyewear products, lenses, services, examinations, or promotional items selected by the Sponsor.
Prize is non-transferable, non-exchangeable, and may not be redeemed for cash or substituted by the winner.
6. TAXES
The winner shall not be responsible for New York State sales tax associated with the awarded prize as determined by the Sponsor. However, winner is solely responsible for any personal federal, state, or local income tax obligations that may arise from acceptance of the prize, if applicable by law.
7. WINNER SELECTION & CLAIM PERIOD
Winner will be selected at random from all eligible entries received.
The selected winner must claim the prize within thirty (30) calendar days from the official announcement or notification date. Failure to claim the prize within the thirty (30) day period shall result in forfeiture of all rights to the prize, and Sponsor reserves the right to select an alternate winner.
Sponsor is not responsible for inability to contact winner due to incorrect or incomplete information provided by participant.
8. PUBLICITY RELEASE
By entering this Sweepstakes, participants grant Michael Alexander Optical Boutique the irrevocable right and permission to use the participant’s and/or winner’s:

Photograph
Video footage
Voice
Likeness
Social media handle and/or testimonial
for advertising, marketing, promotional, public relations, website, email campaigns, social media platforms, print materials, and internal communications without additional compensation, notice, or approval, unless prohibited by law.
9. PRIVACY & HIPAA-STYLE CONFIDENTIALITY

Any information collected during this Sweepstakes shall be handled with strict confidentiality standards and safeguarded in a manner consistent with HIPAA privacy practices and internal confidentiality procedures. Information submitted will not be sold or intentionally shared with unauthorized third parties except as necessary for administration of the Sweepstakes, legal compliance, or authorized marketing purposes agreed upon by the participant.
Participation in this Sweepstakes does not establish a doctor-patient relationship, nor does it guarantee eligibility for medical or vision care services.
10. DISQUALIFICATION
Sponsor reserves the right to disqualify any participant who:
Violates these Terms & Conditions
Attempts to tamper with the entry process
Uses fraudulent methods of entry
Engages in inappropriate, offensive, or unlawful behavior
Attempts to manipulate the outcome of the Sweepstakes
11. LIMITATION OF LIABILITY
By participating, entrants agree to release and hold harmless Michael Alexander Optical Boutique, its owners, affiliates, employees, representatives, and partners from any liability, injury, loss, claim, or damages arising from participation in the Sweepstakes or acceptance/use of any prize awarded.
12. MODIFICATION OR TERMINATION
Sponsor reserves the right to modify, suspend, or terminate the Sweepstakes at any time, with or without notice, if circumstances beyond Sponsor’s control compromise the fairness, integrity, or proper administration of the Sweepstakes.
13. ACCEPTANCE OF TERMS
Participation in this Sweepstakes constitutes full and unconditional agreement to these Terms & Conditions.
